A note to the reader
This was one of my first online projects, which I created in about 2001 when I was a PhD student. The content has never been updated. Even at the time it was an experiment in web presentation rather than a substantial research project. I'm still fond of it, and so it continues to be presented here with the caveat that its references and arguments may now look rather out of date.
